You will receive a Patient Portal Registration email from B-Well Health to create a new patient account.
Charm Health PHR will serve as your patient platform to request appointments, access clinical and visit summaries, request medication refills, view lab and imaging results, send messages to B-Well Health directly, and much more.
On your Charm Health patient portal, select the Documents tab to upload a picture of a valid ID. This is required prior to your visit. For patients under 18 years of age, a parent or legal guardian must upload a valid ID on behalf of the minor patient.
On your Charm Health patient portal, select the Questionnaire tab to complete and submit all current forms. This is also required prior to your visit.
Once all requirements have been met, you are now ready for your visit. Select the appointments tab to access your Telehealth visit at your scheduled time. For voice call visits, please be available at the valid phone number you have provided at your scheduled time.
Though not required, it is helpful to have available any vital sign monitoring devices (blood pressure machine, thermometer) and the most accurate height and weight to assist with providing the best care possible.
Access bwellhealthcare.org to submit your payment. Once payment has been received, any prescriptions needed will be sent electronically to your selected pharmacy. Labs or imaging studies ordered will be available in your Charm Health patient portal.
After your visit is complete you will receive a Patient Feedback questionnaire. Completing this form is not required but will be very useful in helping B-Well Health better service our patients.
